Improving the transition from hospital to home will help create a better system that’s both more efficient and cost-effective and improve the standard of care for seniors, say MIRA members Maureen Markle-Reid and Ruta Valaitis in the Vancouver Sun.
In this op-ed they point out the status quo is not meeting the needs of our aging population and is failing to provide quality care for today’s seniors. They highlight three of their innovative research projects that can improve the transition from hospital-to-home and help create optimal aging at home for older adults with multiple chronic conditions and their family caregivers.
